For the first dropdown, be sure to specify whether or not you have already reached out to us using the form. If you have not, be sure to fill in all of the required information.
At the bottom of the form, you will find an option to upload a video file. Select the option to 'Select Files' and then choose where you will be adding a file from. This will likely be from 'My Computer'.
